The Axial View of the Ankle, also referred to as the Calcaneal Axial View or Axial Heel View, is a specialized radiographic projection used primarily to evaluate the calcaneus (heel bone) and the subtalar joint.

In this view, the patient lies supine or prone with the foot dorsiflexed as much as possible. For the plantodorsal axial projection (the most common type), the X-ray beam is directed from the plantar (sole) aspect of the foot toward the dorsum, angled 40 degrees cephalad (toward the head), and centered at the base of the third metatarsal.

This view is essential for assessing calcaneal fractures, bony spurs, subtalar joint alignment, and posterior heel abnormalities. It offers a clear image of the calcaneal tuberosity, sustentaculum tali, and other structures not well seen in standard AP or lateral ankle views.
